Joseph P. Eaglin, co-founder
UES senior associate, pioneer and nationally recognized specialist in the integration of health and human services programs and systems, helping lead development of provider networks in communities across the country with advanced collaborative technology support through Community Services Network, Inc. (CSN) which he co-founded. He has served as program developer and consultant for national initiatives in a wide range of care programs that range from geriatric support services and senior care advocacy to drug abuse prevention and pediatric asthma. Current consultancy with CMS (Center for Medicare/Medicaid Services) for integrated services and health care reform. Formerly served as chair of the DC Health and Human Services Coalition, and Vice President for Programs of the National Caucus and Center on the Black Age. Other consultancies include Booz-Allen, the Carnegie Foundation, KPMG and Macro International.
Michael D. Mayer, co-founder
UES business and finance associate, has built and run programs and projects of scale in community-based lending, housing, commercial and industrial development prior to collaborative care systems. Specializes in partnering with investors, business and community stakeholders, foundations and government across multiple sectors. Prior to co-founding CSN, set up and ran the Washington, D.C. office of Local Initiatives Support Corporation, which harnesses corporate, philanthropic, and public resources to build community-based solutions for neighborhood transformation. Partnerships he helped forge placed $90MM in successful projects in underserved markets. He has transferred these skills to support UES provider coalition management, care initiatives, financing and resource development.
Hospital, provider practice and fiscal management
A nationally awarded senior healthcare executive with extensive development and administrative experience of major hospital systems with multi-institutional academic affiliations, acute and ambulatory operations. He has served as CEO of the Hurley Medical Center in Flint and the Detroit Medical Center in Michigan, the Providence Hospital in Washington, D.C., The Grady Hospital System and Wesley Woods Geriatric Hospital in Atlanta, Georgia and the New Orleans General Hospital in New Orleans, Louisiana. His experiences in product line development, provider network development and relations and integrated resource management are critical in support of the UES continuity of care – population health model.
UES healthcare physician practice management specialist, with over 20 years of experience in a wide range of practice settings and operations to include single practitioner, large and small group practice, specialty practices, major ambulatory and community care centers. Her expertise in patient flow and clinical resource management, billing, credentialing and insurance contract negotiations, ICD-10 CPT coding, EHR adoption, public and private payer regulatory compliance across the full range of ambulatory care settings is critical to achieving the operational goals of integrated, population health management.
CPA and seasoned healthcare finance management professional, who has served as CFO of numerous medical facilities that include large academic medical centers, regional critical access hospitals, specialty acute care operations, physician-owned as well as community-owned hospitals. He has developed an expertise as a turnaround specialist for financially troubled facilities around the country and leadership in structuring equity financing for a range of healthcare development initiatives.
Healthcare legal specialist and attorney, medical board executive, mediator, medical ethics professor, with a 30 year career in institutional and private practice consulting and leadership. She has served as legal counsel for hospitals, large group practices, managed care organizations, ACO’s, health finance organizations, public health organizations and academic medical institutions. Her extensive healthcare legal expertise is critical to formulating innovative care operations associated with integrated health initiatives.

Mike Foley, PhD
Senior level information technology executive with a record of developing and supporting high ROI IS / IT projects. His core competencies include: HIPAA, Sarbanes–Oxley, servers/telecommunications, team building, BCP, budgeting, staffing, vendor management, service level agreement, voice and data communications, data warehousing, infrastructure (WAN, LAN, VoIP Voice(PBX), security, multi-media) and ERP applications.
He has served as Chief IT Officer for Adventist Health, Samaritan Hospital Systems, and Catholic Health Initiatives. Dr. Foley has also served as CTO of Americas, Operations & Acquisition Integration Leader, GE Energy & Transportation. His extensive experience is critical in creating complex, seamless integrated care delivery networks across highly diverse care delivery operations

Jacquelyn Henry, EdD
Executive leader in social support services and community development, building strategic, collaborative community partnerships, provider training programs and implementing and analyzing policies and initiatives that improve services and outcomes. Founder of the national model Washington D.C. Healthy Families Collaborative, she has a strong background in nonprofit management, and expertise in organizing diverse human and operational resources to support patient-centered care programs.
Senior management experience leading science-based, multidisciplinary communication, policy, and research projects in public and environmental health. Has led the design, conduct, and evaluation of 40 communication, capacity-building, training, research, and stakeholder outreach projects. Currently provides project management and subject matter expertise on critical health issues and sustainable financing, including Medicare and Medicaid, for EPA and other clients.
Founder and principal of a lifestyle improvement and management program used by Mass General Hospital and institutions around the country to teach stress management, healthy lifestyle choices, self-care routines, and lasting behavior change methods. She has an extensive career in hospital – clinical management, nurse training programs and implementation of innovative healthcare initiatives.

Expertise in diagnostic test selection, (including genomic testing and analysis) acquisition and staffing, based on target patients’ needs, includes providing associated workflow integration, billing and compliance.
